Please login to submit an online claim form. You will need your Class Member ID to login. This code can be found on the email that you received in your inbox, or on the postcard that you received in the mail. If you cannot locate your Class Member ID or if you did not receive an email or postcard, please contact the Settlement Administrator at (833) 933-9669. After you login, please provide the requested information, type your name in the signature block, and then click the "submit" button.
After you have successfully submitted your form, you will be taken to a screen that contains your claim number. Please keep a record of your claim number and save for future reference. You will not receive an email confirmation of your submission. If you have any questions or have difficulties using the online form, please contact the Settlement Administrator.
If you wish to mail in your Claim Form instead of submitting it online, you may print, complete, and mail a paper Claim Form available for download in the documents section of this website.
