Please login to submit an online Opt-Out Form. You will need your Class Member ID to login. This code can be found on the email that you received in your inbox or on the postcard that you received in the mail. If you cannot locate your Class Member ID or if you did not receive an email or postcard, please contact the Settlement Administrator at (833) 933-9669. After you login, please provide the requested information, type your name in the signature block, and then click the "submit" button. You will not receive an email confirmation of your submission. If you have any questions or have difficulties using the online form, please contact the Settlement Administrator.
If you wish to mail in your Opt Out Form instead of submitting it online, you may print, complete, and mail an Opt Out Form available for download in the documents section of this website.
